Passport Exemptions include:
" Holders of a valid on arrival Form I-512 ( Authorization for Parole of an Alien into the United States").
" Holders of a Temporary or Emergency passports.

Visa is required
Visa exceptions:
" Travelers wishing to enter or transit through the USA under the Visa Waiver Program (VWP) must apply for authorization to travel using the Electronic System for Travel Authorization (ESTA). It is advised to apply for the ESTA at least 72 hours before departure.

Destination information is required
Passengers traveling to the U.S. must provide a full destination address (including zip code) for their first night’s stay in the U.S.. Failure to provide this may result in a passenger being refused entry to the U.S. and not being accepted for travel at check-in.

Travelers who arrive at check-in without a full destination address or without any pre-booked accommodation, will be required to pursue this matter with U.S. immigration.
"Passengers transiting to a cruise ship will need to provide:
Name of the cruise ship you will be transiting to, city, state, zip code if known.

"Passengers picking up a rental car need to provide:
General itinerary if the address for the first night’s stay is not known.

"Passengers who do not know their hotel's address may be allowed to provide:
Name of the hotel, city, state and zip code if known.

Exemptions to destination information provisions include:
" U.S. citizens
" U.S. lawful permanent residents
" Passengers in transit to other countries (staying fewer than eight hours in the U.S. and have confirmed onward reservations.)
